Job Description

Coordinates Initial Plan of Care: Facilitates communication, coordination and problem-solving with physician, patients, families and staff to develop initial infusion plan of care. Works with multiple disciplines from pharmacy, care management and outside agencies to establish an appropriate and cost-effective plan.

 Provides ongoing Care Coordination and Scheduling: Responsible for overseeing the daily coordination of workflow and scheduling of nursing care in the delivery of services. Works with primary RNs and patients to assist in managing scheduling issues.

 Serves as clinical expert nursing consultant to hospital liaisons, discharge coordinators and community agencies to assure continuity of care for patients transitioning between providers. Participates in marketing and promotional activities. Maintains a high degree of visibility with the staff, patients, referral sources and supporting institutions.

 Participates in the delivery of direct patient care activities as necessary. Maintains in-depth nursing knowledge and clinical expertise in the specialty of Infusion.

 Assures nursing responsibilities are met for authorization from payors. Works with support personnel to facilitate admission process. Works in cooperation with the Authorization staff to assure appropriate information is available regarding authorization for services.

Skills and Requirements

3 years home infusion, acute care or home health RN experience

Customer Service experience

OR State RN License BSN null
