Job Description
A healthcare client in San Diego is looking for a Social Worker to join their team. The Social Worker will be responsible for social work case management services to the participants of the San Diego PACE program. They will provide services such as participant screening, case management, counseling, and referrals. They will provide in-home assessments as needed. This position is M-F, 8 AM to 5 PM. They will float between Chula Vista/San Ysidro or El Cajon/Vista locations.
